What Is a Deep Plane Facelift?

A deep plane facelift is an advanced surgical technique that repositions the deeper layers of facial tissue to achieve natural-looking and long-lasting results. It is ideal for men and women with advanced facial aging looking for the most significant improvement.

Technique

Your plastic surgeon targets the underlying structures of the face (including the muscle and fat). Beyond tightening the skin, this technique allows for more extensive lifting and tightening of the mid and lower face for a significantly refreshed look. Most patients are in search of the less stretched, more natural and rested appearance that is typical of a deep plane facelift.

Results

The deep plane facelift is known for its superior, natural-looking results and striking before and after transformations. This approach addresses multiple signs of aging, such as nasolabial folds, marionette lines, sagging cheeks, and heavy jowls.  It is common that patients may also experience improvements in volume under the eyes and into the forehead and temple, especially when performed with volume fat transfers and temple brow lifts.

Recovery

With a deep plane facelift, the deeper tissues need to heal, but surprisingly do not result in a longer recovery period compared to traditional facelifts. Most patients can return to normal activities within 2 to 3 weeks. While many patients feel physically well sooner, they may feel like they need slightly more social downtime before they are comfortable being seen in public.

What Is a SMAS Imbrication Facelift (Traditional Facelift)?

A SMAS (superficial musculoaponeurotic system) facelift lifts and tightens both the skin and the underlying SMAS layer, which includes muscles and connective tissue. It is an effective solution for moderate signs of aging. 

Technique

A traditional SMAS facelift uses the same incision locations as a deep plane facelift, but it only repositions the skin and SMAS layer. The dissection is less extensive than the deep plane approach, and the SMAS and platysma serve as a girdle to compress the underlying tissue and tighten the skin.

While this procedure typically addresses the lower face and neck, improving jowls, loose skin, and deep wrinkles, it employs a pulling mechanism to achieve results rather than a softer volume repositioning maneuver typical of a deep plane facelift. In other words, a traditional SMAS facelift is more about stretching and tightening than tissue manipulation.

Results

A traditional SMAS facelift results in a refreshed, more vibrant, and youthful appearance. By targeting the SMAS layer, this facelift method can achieve natural and long-lasting results, though not as extensive as those from a deep plane facelift.

Recovery

Recovery from a traditional SMAS facelift involves healing of the skin and underlying SMAS layer. Patients usually experience swelling and bruising, with most returning to normal activities within 2 weeks. Learn more about facelift recovery on our dedicated page.

Both deep plane and traditional (SMAS) facelifts offer significant benefits, but the choice depends on your specific needs and goals.
